The report claims healthcare systems billed nearly $120 million for gender-affirming care for young patients between 2019 and 2023, while subjecting thousands of minors to irreversible medical procedures.

Focus on Detransitioners and Medical Consent
The document incorporates testimonies from several „detransitioners”—individuals who halted or attempted to reverse gender-transition treatments started during adolescence.
One case highlighted in the report involves Clementine Breen, who began receiving puberty blockers, testosterone, and a double mastectomy at Children’s Hospital Los Angeles starting at age 12. Breen later stopped hormone therapy at age 18, pointing to unaddressed trauma from past sexual abuse as the root cause of her identity distress. The report criticizes medical providers for failing to adequately evaluate underlying psychological factors or properly warn minor patients about long-term risks, including loss of fertility.
Federal Criminal Investigation Launched
Following the release of the report, the Trump administration announced that it is opening a formal federal investigation into the healthcare systems and clinics named in the document.
Vice President JD Vance announced on social media that he formally referred the matter to Attorney General Todd Blanche and the Department of Justice to investigate potential health care fraud, insurance fraud, and federal law violations.
The move marks a complete reversal from the policy approach under the Biden administration, which had utilized Section 1557 of the Affordable Care Act to protect gender-identity-based care and prohibit discrimination against patients seeking gender-affirming treatments.
